ECG Abnormalities_Part 17

A 21 year old healthy female comes to the clinic with complaints of intermittent palpitations. These episodes can last from seconds to minutes and are associated with lightheadedness, diaphoresis, and occassionally chest pain. Her ECG is below:

    Unregular Sinus Rhythm, rate about 65 beats/min
    Normal Axis
    Wolff–Parkinson–White syndrome

    ECG 2:
    Paroxysmal Supraventricular Tachycardia, rate about 200beats/min

  18. ECG #1 is definately WPW with a short PRI and delta wave.

    ECG #2 however is a 2:1 Atria Flutter in which the flutter waves are clearly seen in the precordial leads of V2 - V6. this would explain the symptoms of C/P, SOB, dizziness and palpitations. With such a rapid heart rate, the heart has a shorter time to preload during diastole, thus reducing CO.
